Job Description:
The C. W. WIlliams Community Health Center, Inc. is expanding
and looking for a Case Manager to join our team. This position will
provide a range of client-centered, confidential services that link
clients with core care services.*ESSENTIAL FUNCTIONS AND
RESPONSIBILITIES:** Assessing patient needs: Needs assessment is
both initial and ongoing.* Creates care plans that match the
identified needs of the client* Ensures all clients referred to
services are eligible prior to the date of service* Listening to
clients with compassion* Conducts outreach services as needed*
Refers clients to appropriate services highlighted in the care
plan* Screens for income supports for benefits/entitlements to
advocate on behalf of client* Completes emergency financial
assistance applications for eligible clients* Completes payment
requests as needed for copays and other out of pocket costs*
Documents encounters using designated client database accurately
and in a timely manner* Develops and maintains professional
relationships with social service, health and governmental
agencies* Participates in continuous quality improvement efforts*
Participates in regular supervision meetings, team meetings, in
house training sessions and conferences as needed* Protects
organization's value and manage risk by keeping information
confidential* Performs other duties as assigned*EXPERIENCE AND
EDUCATION:** Associates degree in any Human Services field with at
least four (4) years of case management experience* Bachelors
degree in any non-human services field with at least two (2) years
of case management experience, preferred* Bi-lingual, preferred*
Basic knowledge and understanding of the Case Management activities
and care components of Case Management defined in the Methods and
Techniques section* Knowledge of Behavioral Health Case Management
theory and techniques required* Requires the ability to work as
part of a dynamic, professional health care team* The ability to
maintain accurate work records and access these records as
necessary.* The ability to use computer and web-based systems
(e.g., PC-based tools, Microsoft applications, Web-based
