We want everyone who works at CVS Health to feel empowered by the role they play in transforming our culture and accelerating our ability to innovate and deliver solutions to make health care more personal, convenient and affordable.Position SummaryThis Social Worker position is telework/hybrid and must be within driving distance of the High Point, North Carolina office.Normal Working Schedule: Monday -Friday 9am-5:30pm EST with flexibility needed for member calls after 5pm.0-10% travel possible including but not limited to travel to the High Point office for customer meetings .The Social Worker partners with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Responsibilities include researching available options and resources for members with financial, transportation, safety, and housing needs.The Social Worker will work with a population spans the entire lifecycle.All member contact is done telephonically or via electronic exchange.The Social Worker's role is to empower members to make positive lifestyle and medical choices and works as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.Required QualificationsMust live in the state of North Carolina-Candidate is required to have licensure: Licensed Masters Level Social Worker/Certified Master Social Worker (LMSW/CMSW) or Licensed Clinical Social Worker ( LCSW)-Must have an unrestricted active Social Worker license in the state of North Carolina-If applicable to engage members in other states, must be willing and able to obtain multi-state licensure (additional licensures will be paid for by the company).-2+ years' experience in medical social workPreferred Qualifications-Telephonic Social Worker experience-Advanced care planning experience-Hospice/palliative care experience-Case management and discharge planning experience-Managed care/utilization review experience-Crisis intervention skills-Ability to exercise independent and sound judgment in decision making-Highly organized to manage continuously changing priorities-Proficiency with computers and an ability to navigate multiple systems simultaneouslyEducationMaster's Degree Licensed Social WorkerPay RangeThe typical pay range for this role is:$42,312.40 - $91,100.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
