Description:Purpose of the Position: The Hospice LPN plays a
vital role in delivering compassionate, high-quality care to
patients at the end of life. Working under the direct supervision
of a registered nurse, the Hospice LPN is responsible for
implementing the plan of care, providing hands-on nursing support,
and documenting all services accurately and promptly. The Hospice
LPN collaborates closely with the interdisciplinary team to ensure
the physical, emotional, and spiritual needs of both patients and
families are met with excellence and dignity. This role requires
keen assessment skills, attention to detail, and a deep commitment
to patient-centered care. The Hospice LPN also ensures effective
communication with the supervising RN and all care disciplines,
promoting continuity and quality of care throughout the hospice

MedicineEssential Responsibilities:Provide high quality patient
care consistent with the patient's plan of care, agency policies
and procedures, and coverage/reimbursement guidelines, consistent
with the Nurse Practice Act in the state where care is given. This
includes direct treatment; reinforce teaching, and follow-up care
to patients as assigned by supervising RN.Prepares clinical visit
notes and follows patient care plans as developed by the registered
nurse and according to agency policy.Documents and ensures
appropriate use of medications, DME and suppliesProvides education
to patient, families and/or others in care techniques specific for
patientReports any significant change or adverse reaction to
treatment to primary nurse; modifies plan as ordered by the
physicianConsults with the primary nurse regarding needs for
additional servicesAttends educational programs to learn and review
information necessary to provide patients with quality
careParticipates in IDG meetingsMaintains confidentiality in all
aspects of the jobAssesses signs and symptoms indicating
physiologic and psychosocial changes in the patient's
condition.Communicates consistently and appropriately with
patients, families, physicians, appropriate health professionals
and team members.Promotes and maintains patient quality of life
under the guidance of the registered nurse.Participate in
department or organizational Quality Improvement
activities.Participates in on-call rotation.Willing to travel to
Hospice coverage areas for care delivery.Performs other duties as
requestedRequirements:Qualifications:Graduate of an accredited
school of Nursing, current LPN license required, one year of
clinical nursing experience, prefer home health or hospice
experienceStrong organizational, communication, listening, and
assessment skills CPR certification requiredTb Skin Test within the
last yearKnowledge of hospice guidelines and be able to read and
interpret guidelines, policies and proceduresProfessional phone and
interpersonal behavior; Respect confidentiality of patient
dataUnderstanding of medical terminology and calculationsMinimum of
eight (8) hours of continuing education annuallyReliable
transportation Physical Demands:Sitting for various lengths of time
while operating the computer with frequent stretching, talking,
hearing, smiling, bending, and reaching. The ability to stoop or
kneel, bends, lift, push, reach with hands/arm, and smell when
providing patient care. Must be able to walk up stairs in patient
residences. Requires moving supplies and/or equipment from vehicle
to patient's residence.Compensation details: 20.66-29.45 Hourly
