Responsibilities About the Role During visits that may last up to one hour, our providers assess a member’s current health status, medical history, medication compliance, social circumstances, and other potential risks. This approach provides invaluable insights into a member’s overall health that are often missed during standard office visits. The Comprehensive Health Assessment (CHA) aims to enhance care quality and potentially address multiple care gaps in a single visit. Our providers: Perform assessments for adults and geriatrics, including medical history review, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and thorough documentation in accordance with our guidelines. Administer and order necessary point-of-care and laboratory tests. Collaborate with physicians, case managers, social workers, family members, primary caregivers, and other medical personnel as needed. Engage with Primary Care Physicians (PCPs) for patient education and follow-up as required. Deliver services at the member’s residence, whether in their homes or in Skilled Nursing Facilities. Benefits of Joining Our Team Flexibility – Schedule visits according to YOUR availability. Assessments are conducted from Sunday to Saturday, between 8 am and 7 pm. You can choose to work during the day, evening, or weekends based on your preferences. Competitive Pay – Your earnings are based on the number of assessments completed. Increase your income potential by being available for more visits and traveling to see members outside your home area. Those averaging 20 to 100 visits monthly can earn between $1,800 and $11,500 , with assessment rates ranging from $90 to $115 depending on your state. Additional Benefits – Includes mileage reimbursement, a 401(k) plan, employee referral bonuses, and limited voluntary benefits. Education and Support – We prioritize your development! Along with comprehensive onboarding training, you will be assigned a mentor, receive personalized feedback, and have free access to continuing education opportunities. Licensure Support – If business needs arise, we will assist you in obtaining additional state licensure and credentialing in neighboring states or others you may wish to visit while conducting health assessments. Full-Time Opportunities – PRN providers have the option to transition to full-time roles supporting their home location or a specific state or region, along with eligibility for a full-time sign-on bonus. Qualifications Skills & Experience That Stand Out A Master’s Degree or equivalent experience, along with successful completion of NP licensure. Current NP licensure in the state of practice, including prescription authority or the ability to obtain it as necessary. AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty. Current BLS, ACLS, or CPR certification. Preferred one year of NP experience; new graduates are encouraged to apply! A valid state driver’s license and proof of adequate automobile insurance for your state of residence. Proficient computer skills and familiarity with health/medical record software. Excellent verbal and written communication abilities.
