ResponsibilitiesAbout the Role During visits that may last up to
one hour, our providers assess and evaluate a member’s current
health status, medical history, medication adherence, social
circumstances, and other potential risks. This process offers
invaluable insights into a member’s overall health and well-being,
insights that can be challenging to obtain during standard office
visits. The Matrix Comprehensive Health Assessment (CHA) aims to
enhance care quality and allows us to address multiple care gaps in
a single visit. Our providers are responsible for: Conducting
assessments for adults and geriatrics, which include reviewing
medical history, diagnoses, treatment plans, health education,
physician referrals, case management referrals, follow-ups, and
thorough documentation in line with our guidelines and protocols.
Administering and ordering necessary point-of-care and lab-based
tests. Collaborating with physicians, case managers, social
workers, family members, key caregivers, and other medical
personnel as appropriate. Working alongside Primary Care Physicians
(PCPs) for patient education and necessary follow-ups. Providing
services at the member’s residence, which may include their homes
or Skilled Nursing Facilities. Benefits of Working with

We conduct assessments from Sunday to Saturday, between 8 am and 7
pm. You can choose to work during the day, evening, or weekends
based on your personal schedule. Competitive Pay Your earnings will
be based on the number of assessments you complete. You can
increase your earning potential by being more available and
traveling to see members outside your home area. Those who complete
an average of 20 to 100 visits per month can earn between $1,800
and $11,500 monthly, depending on the per assessment completion
rate of $90 to $115 , which varies by state. Additional Benefits

Licensure If there is a business need, we will assist you in
obtaining additional state licensure and credentialing in
neighboring states or others you may wish to visit while conducting
health assessments. Full-Time Opportunities Our PRN providers have
the option to transition into full-time roles supporting their home
location or a specific state or region, along with eligibility for
a full-time sign-on bonus. QualificationsSkills & Experience That
Shine Master’s Degree or equivalent experience, along with
successful completion of NP or PA licensure. Current NP or PA
licensure in the state of practice, including prescription
authority or the ability to obtain it. Board certification from
NCCPA, AANP, ANCC, or AACCN in Adult, Adult Gerontology Primary
Care, Adult Gerontology Acute Care, Family, Emergency, or Acute
Care specialty. Current BLS, ACLS, or CPR certification. Preferred
one year of experience as a Nurse Practitioner or Physician
Assistant; new graduates are encouraged to apply! Valid state
driver’s license and proof of adequate automobile insurance for the
state of residence. Strong computer skills and familiarity with
health/medical record software. Excellent verbal and written
