Job Description:
MindPath Care Centers, a pivotal division of Community
Psychiatry, is extending an opportunity for a dedicated Outpatient
Hybrid Psychiatric Mental Health Nurse Practitioner (PMHNP). Our

outpatient mental healthcare, emphasizing personalized therapy and
medication management. The role offers a dual-channel of engagement
with patients through both virtual and in-person interactions,
catering to a diverse demographic in need of precise and adaptive
psychiatric care. Candidates will have the chance to work
autonomously yet collaboratively within interdisciplinary teams in
accordance with the highest standards of mental health practice.
Conduct thorough psychiatric evaluations and determine appropriate
treatment plans. Provide medication management tailored to
individual patient needs. Engage in both virtual and face-to-face
sessions with patients, ensuring continuity and quality of care.
Coordinate with other medical and psychological professionals to
optimize patient treatment plans. Document treatment and patient
progress comprehensively and timely within the electronic
healthcare record system. Educate patients and families about
mental health conditions and treatment protocols. Stay abreast of
the latest developments in psychiatry to implement evidence-based
practices. Proficiency in psychiatric evaluation techniques. Strong
ability in pharmacological management and patient counseling.
Excellent communication and interpersonal skills. Ability to manage
a flexible schedule that includes virtual and in-person patient
visits. Skilled in the use of electronic medical records and
telepsychiatry platforms. Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Certified Psychiatric-Mental Health
Nurse Practitioner (PMHNP-BC). Valid RN and APRN licenses in the
state of practice. DEA registration and eligibility to prescribe
and manage medications.
