Job Description:
H2 Health is seeking a dedicated and compassionate Physical
Therapist to join our multidisciplinary team at our
state-of-the-art facility. We pride ourselves on offering
comprehensive rehabilitation services including physical,
occupational, and speech therapy, tailored to meet the unique needs
of our diverse clientele. As a Physical Therapist at H2 Health, you
will play a crucial role in helping patients restore, maintain, and
promote optimal physical function and wellness. Assess patient
condition by reviewing their medical history and any referrals or
notes from doctors and surgeons Formulate treatment plans to
address the needs of patients, including pain relief, muscle
strengthening, and overall physical functioning Implement
therapeutic interventions for patients using manual therapy
techniques, therapeutic exercises, and specialized equipment
Document patient care services by charting in patient and
department records Guide and mentor physical therapist assistants
and support staff to ensure quality treatment Evaluate effects of
therapy and adapt treatment plans as necessary Educate patients and
family members about treatments, exercises, and injury prevention
Maintain patient confidence and protect operations by keeping
information confidential consistent with HIPAA regulations Strong
knowledge of physical therapy principles, theory, and practices
Excellent patient assessment and clinical reasoning skills
Proficiency in using physical therapy tools and equipment Strong
interpersonal and communication skills Ability to work
collaboratively in a multidisciplinary team Medical Insurance

Association's BLS for Healthcare Providers certification
