CHESS: Director of Pharmacy

Reports to: Chief Medical Officer

Reporting to this position: Clinical Pharmacy Specialists, Pharmacy Technicians

FLSA Status: Exempt

Position Summary:

The Director of Pharmacy will direct and/or supervises clinical and support staff in the delivery of clinical pharmacy services. This position is critical to the CHESS mission to deliver value for our clinical partners. The Pharmacy Director will perform the duties of a Clinical Pharmacist Practitioner who will work with the physicians and staff of various CHESS Value Partner practice sites to provide pharmacology education, disease state education, direct patient care, and medication management to improve the health status of the patients served.

Position Responsibilities:

Evaluates performance of clinical and support staff and coordinates recommendations for merit increase

Coordinates hiring of all clinical and support staff with Human Resources

Recommends staff disciplinary actions

Oversees and directs all patient clinical services

Performs monthly review of financials

Monitors staff productivity and directs staff assignments

Ensures adequate staff coverage through scheduling

Recommends new services and generally plans for strategic growth

Monitors quality of all clinical and patient services

Set policies and procedures of clinical services

Fosters growth through marketing efforts

Represents needs of CCPS to senior management

Sets annual budget

Ensures all regulations and accreditation standards are met

Monitors physician satisfaction with clinic services

Evaluates and improves operational efficiencies of clinic

Provides patients who are referred to Clinical Pharmacy Services with the appropriate education regarding the disease state for which they are referred and assist in choosing appropriate medical therapy when needed

Collaborate with providers to individualize drug therapies promoting safe, effective, and economical patient care

Assume responsibility for monitoring and modifying the drug regimen when referred

Act as a drug information resource to providers, nursing staff and patients

Creates and maintains pharmacology protocols for disease state management office visits for diabetes, poly=pharmacy, lipids, and anti-coagulation, as well as any other disease states as needed

Serves as preceptor for pharmacy students when applicable

Provides extensive chart review and one-on-one meetings with patients and provide verbal and/or written feedback to providers

Participates in collection of quality management data and document outcomes as appropriate

Utilizes the results of data to initiate change in clinical practice in department

Addresses risk management issues regarding safety and poly-pharmacy issues

Enhances professional growth and development through in-service meetings, educational programs, and conferences, and participates in committees as requested

Actively participates as a team member by supporting decisions, accepting change, managing conflict effectively and valuing the contributions of others

Fosters and environment of mutual respect with special concern for confidentiality and the privacy of others

Communicates effectively and courteously on the telephone and in person with patients and colleagues

Position Qualifications: Required

Graduate of accredited school of pharmacy

Bachelor of Pharmacy or Doctor of Pharmacy Degree

At least 1 year of clinical direct patient care experience

Current state pharmacy license or have met the following requirements to apply for CPP license: Have completed a Board of Pharmaceutical Specialties Certification or Geriatric Certification, or American Society of Health-Systems Pharmacist accredited residency program and have 2 years clinical experience OR

Have earned a PharmD degree, have 3 years’ experience and have completed a Certificate Program OR

Have earned a BS degree, have 5 years’ experience, and have completed two certificate programs

Skills, Knowledge and Abilities Skill in producing statistical reports needed by clinic, reimburses and regulators Skill in appropriately counseling patients, families, and health care staff about medications

Skill in organization and attention to detail

Skill in using customer service techniques

Knowledge of current pharmaceuticals, prescribing patterns, new therapeutic technologies

Knowledge of third-party reimburses and managed care requirements including using computers to access/enter data

Knowledge of computer systems and applications

Knowledge of organizational policies, procedures, and protocols

Knowledge of management and leadership principles including program development, implementation, and evaluation. Knowledge of common safety hazards and universal precautions to establish a safe work environment

Ability to work effectively as a member of health care team

Ability to communicate clearly and accurately and work well with others

Ability to reach calmly and effectively in emergency situations

Ability to identify problems and implement solutions

Ability to demonstrate compassion and caring in dealing with others

Access to Protected Health Information (PHI) This position requires the employee to handle PHI for duties related to enrollment and eligibility

issues. Policies and Procedures will be strictly adhered to make sure PHI is protected as required by the HIPAA Privacy Rule.

Privacy and Security Responsibilities:

Responsible and accountable for maintaining the confidentiality, integrity, and availability of protected health information. Follow HIPAA security policies and procedures affecting your job and report any suspected or actual violation or breach.

Essential Physical Functions:

This position operates in a professional office and remote environment. This role routinely uses standard office equipment. While performing the duties of this position, the employee is occasionally required tostand; walk; sit, push, pull; use hands to finger, handle, or feel objects, tools or controls, reach with hands and arms; climb stairs; talk and/or hear.
