Job Description:
Provides skilled homecare services to children with chronic
health conditions, ensuring personalized one-on-one care in
patients' homes.
Maintains accurate clinical records, evaluates care effectiveness,
and consults with physicians to adjust care plans.
Mentors new staff, educates families, and participates in quality
improvement programs.
Requires an unencumbered LPN/LVN license, at least 1 year of
experience, CPR certification, and a valid driver--------s license
(varies by state).
Performs duties in home settings, with physical requirements
including lifting up to 35 pounds.
